Meet Noemi — Perinatal Stroke

1 – Purtroppo ci hanno confermato l’ictus perinatale solo quando la mia Noemi aveva 13 mesi abbondanti.

2 – Fino al giorno della diagnosi non abbiamo mai avuto la necessità di recarci in un pronto soccorso poiché non ha mai manifestato sintomi così gravi.

3 – Non ci sono mai stati prescritti farmaci.

4 – La nostra storia è iniziata esattamente il giorno in cui compiva il suo primo anno , non dimenticherò mai quella data……..da quando la mia bimba aveva 9 mesi avevo segnalato più volte alla pediatra che secondo me qualcosa non andava poiché avevo notato come utilizzasse diversamente la mano destra rispetto alla sinistra.

Nonostante i miei dubbi la pediatra mi rassicurava asserendo che in quei mesi è normale che la utilizzino poco e che probabilmente era mancina, i mesi passavano ma la mia bimba continuava a tenere la mano destra chiusa a pugno così tornai dalla pediatra la quale si decise a prescriverci una visita neurologica urgente consigliandoci la neurologia pediatrica dell’ospedale Buzzi. Contattai subito l’ospedale per fissare una visita e il giorno in cui compiva 1
anno ci presentammo in ospedale, la dottoressa visitò Noemi e poco dopo ci disse che secondo lei la bimba aveva un’emiparesi destra!

Prima di lasciare lo studio ci diede l’appuntamento per la risonanza magnetica, così dopo circa un mese e mezzo la diagnosi fu confermata: emiparesi destra, i medici ci spiegarono che molto probabilmente vista l’entità della lesione e ciò che riesce a fare Noemi è molto probabile che l’ictus si sia verificato quando ancora era nell’utero, purtroppo la conferma non potrà mai darcela nessuno. In seguito Noemi fu subito presa in carico dalla fondazione Don Gnocchi dove è attualmente seguita, pratica fisioterapia, idroterapia e logopedia, ogni 3 settimane una seduta dall’osteopata.

Privatamente la sottoponiamo a trattamenti mensili di crosystem, fino ai 3 anni ogni 6 mesi ha eseguito l’elettroencefalogramma in sonno, dai 3 anni però solo una volta all’anno, ad oggi non ha mai manifestato crisi.
A Noemi sin da quando aveva due anni è stato prescritto l’utilizzo di un tutore AFO e di scarpe ortopediche.

Riguardo la mia gravidanza nulla da dire, 36 anni, aumento ponderale durante la gravidanza : 10 kg , nessun problema di   salute e il decorso è stato regolare. Noemi secondogenita è nata a 41+3 con parto naturale  ma indotto, un travaglio durato più di 24 ore, Noemi pesava Kg 3.690 per 53 cm di lunghezza, allattata per 6 mesi presentava un’ottima suzione.

Sia io che mio marito ci siamo sottoposti a test genetici che non hanno evidenziato nessuna mutazione, l’unico esame dal quale è emerso un dato al di sotto dei valori minimi è la proteina S e libera, sia in me che in Noemi questo valore è basso, i medici hanno detto che Noemi dovrà ripetere questo esame nella pubertà.
Alla nascita godeva di ottima salute, unico particolare è che ci hanno dimesse in quinta giornata (insolito per un parto naturale), alle dimissioni la bimba presentava uno strabismo ma l’oculista mi disse che poteva capitare e che nel giro
di un mese gli occhi si sarebbero riallineati……..infatti così è stato !

1 – Unfortunately we had confirmation of perinatal stroke diagnosis only when Noemi was 13 months.

2 – Until the day of diagnosis we never had had the need to bring her in an emergency room as she had never experienced such severe symptoms.

3 – There have never been prescribed medications.

Our story started the day of her first birthday, we will never forget that date. When my daughter was 9 months I had reported to the pediatrician several times that I thought something did not go well; I noticed how she used only the left hand and not the right.

Despite my doubts, the pediatrician reassured me, asserting that in those months such behavior is normal and that probably she was left-handed. Months went by, but my daughter was still holding her right fist, so I went back to the pediatrician who decided for her to take an urgent neurological examination, at Buzzi pediatric hospital (in Milan).

I immediately contacted the hospital to arrange a visit and on her first birthday we went to the hospital. The doctor visited Noemi and shortly after we were told that the child might be affected by right hemiparesis!

Before leaving the hospital, we had an appointment set for magnetic resonance imaging, so after about a month and a half, diagnosis was confirmed: right hemiparesis. They explained to us that, given the size of the injury and what Noemi can do, it is very likely that the stroke occurred when she was in the womb, unfortunately no one will ever confirm.

In the following, we brought Noemi at the foundation Don Gnocchi where she could practice physical therapy, hydrotherapy and speech therapy, and osteopath sittings. Privately Noemi also underwent monthly CroSystem treatments, up to 3 years’ age every six months she has taken electroencephalogram in sleep. From 3 years’ age, however, only once a year; to date she has never expressed crisis.

Noemi, since she was two years, has been prescribed the use of AFO tutor and compatible orthopedic shoes.

Regarding my pregnancy, I have nothing exceptional to say, 36 years, weight gain during pregnancy: 10 kg, no health problem and the course was uneventful. Noemi was second child, born at 41 + 3 but induced with natural childbirth, that lasted more than 24 hours. Noemi weighed 3,690 Kg to 53 cm in length, breast fed for 6 months and showed an excellent suction.

Both my husband and I were subjected to genetic testing that identifies no mutation, the only test which showed a figure below the minimum values is the free protein S, both for me and Noemi this value is low. The doctors said that Noemi will have to repeat this test at puberty.

At birth, she was in excellent health, especially unique is that we left the clinic on the fifth day (unusual for a natural birth). The child presented a squint but the eye doctor told me it could happen, and that within a month eyes would be realigned …….. indeed, they did!
